RWU Animal Event on Campus Request Form

Please complete the following form to request an animal event on campus. Click the arrow button at the bottom of the page under the signature box to electronically submit the request. Please save a copy of the completed request (will automatically direct you to that page after submission) for your records.
 
Please submit at least 30 days in advance of your event. Please do not sign contracts or put down deposits until the event has been approved by EHS.

Provider's USDA License for Animal Exhibitions (if applicable)
