All students enrolled in one or more Fall 2021 MNS laboratory classes must review the following information and agree to follow the laboratory safety requirements to participate in laboratory classes. Please fill out all required sections and submit your completed agreement electronically to meet this requirement. Please download a copy of your completed survey once it has been submitted (link will appear on the screen). Please reach out to your laboratory instructor with any questions or concerns.
